How does Modulen IBD Work?
Modulen is a nutritionally complete feed which contains TGF-ß2 which is an anti-inflammatory cytokine which helps to down-regulate the inflammatory response associated with active Crohn’s disease.

Why should Modulen IBD be used for 8 weeks?
Modulen IBD should be used as a sole source of nutrition for 8 weeks to allow time for mucosal healing and down regulation of pro-inflammatory cytokines. Shorter periods of 4-6 weeks are currently in practice but mucosal healing with investigations have not been studied.

Can I flavour Modulen IBD?
Modulen IBD has a natural flavour. We have flavour pots in 5 different flavours that are available on prescription for use with Modulen IBD. It is possible to add other flavouring such as nesquik, decaffeinated instant coffee powder or crusher but always discuss with your Dietitian or Dr beforehand.

Is Modulen IBD suitable for vegetarians?
Yes, but not suitable for vegans as it contains milk protein

Can you cook with Modulen IBD?
Modulen IBD should not be used in cooking as this will denature the protein and render TGF?2 inactive.

Can you freeze Modulen IBD?
Yes. Modulen can be frozen.

How long can Modulen IBD be used after it is reconstituted?
Refrigerate and use within 24 hours or 4 hours if at room temperature.

Is Modulen IBD nutritionally complete?
Yes.

What age groups can Modulen IBD be used in?
The nutritional content of Modulen IBD makes it suitable for paediatric and adult patients. Modulen IBD should be used with caution in children under five years and is not suitable for infants under one year. If patients are under 5, consider using Peptamen Junior which is suitable for 1-10 years

Is Modulen IBD prescribable?
Yes, Modulen IBD is prescribable as a sole source of nutrition during the active phase of Crohn’s Disease and during the remission phase as a nutritional supplement.

Can Modulen IBD be used jointly with medication e.g. steroids?
Yes, patients can take medication with Modulen IBD, but this should only be done under the supervision of their Doctor

Can Modulen IBD be used in patients with Ulcerative Colitis?
There is no clinical evidence to support the use of Modulen IBD in patients with Ulcerative Colitis, although it is increasingly being used by Paediatric centres.

Does breastmilk/ cows milk/other formulas contain TGF-B2?
TGF-B2 is naturally present in milk. Modulen IBD provides a concentrated form of TGF-B2 above levels found naturally in milk. Other formulas do not contain TGF-B2 because they may not be casein based or because heat treatment has destroyed the TGFb2

Is Modulen IBD suitable for kosher diets?
Modulen IBD does not contain any ingredients, which would not be permitted, however Modulen IBD is not Kosher Certified

Why is Modulen IBD not available as a liquid?
The heat treatment required to make the liquid would destroy TGFb2

Is Modulen suitable as a sole source of nutrition during pregnancy & Breastfeeding?
Yes, the amount consumed will have to be increased to meet extra energy and nutrient requirements and this will depend on the individual patient.

How long can Modulen be left in an enteral reservoir once reconstituted?
Maximum hanging time is 4 hours or check with local policy

What is the GM position on Modulen IBD?
Modulen IBD does not contain DNA/ protein from GM Soya and/or maize. Any products containing Soya are manufactured from stocks of non-GM Soya protein and maize starch.

How much does a scoop of Modulen weigh?
1 level scoop = 8.2g

Is Modulen IBD nut free?
Yes it is.

What other liquids apart from Modulen IBD are allowed?
This should be according to local policy or as advised by the dietitian.

My patients have started on Modulen IBD and their stools are green! Is this normal?
Yes, an infection needs to be excluded however, biliverdin is a precursor of bilirubin which is metabolised by the liver. In a acute disease or genetic condition, the liver fails to get rid of these waste products and the biliverdin builds up in the body and stains stools green (or yellow)

What are the added sugars in Modulen?
Sucrose & Corn syrup

Peptamen

Which type of Peptamen should I add the flavour pots to, vanilla or unflavoured?
Peptamen flavour pots should be added to the Vanilla cup to increase the flavour range and compliance. The unflavoured can is for use as a tube feed.

Why can only 10 cups of Peptamen be consumed per day?
It is recommended that no more than 80g proteins are consumed per day (i.e. 10 cups of Peptamen). However it is up to the dietitian to decide on an individual patients requirements.

What percentage of fat in Peptamen is present as MCT?
70%. This allows a rapid absorption of fat and is an immediate source of energy.

Is Peptamen suitable during pregnancy and breastfeeding?
Yes. The daily intake of Peptamen will need to be increased to meet increased energy and nutrient requirements

Are there any artificial flavourings in Peptamen?
The vanilla flavour in Peptamen is natural.

What is the GM position for Peptamen?
Peptamen unflavoured and flavoured are manufactured in Europe using maize starch from European non-GM stocks.

Can it be frozen?
This is not recommended.

Is Peptamen suitable for severe milk allergy?
No, as it contains milk protein which is not hypoallergenic.

In what volume is Peptamen nutritionally complete?
1500ml.

In what volume is Peptamen HN nutritionally complete?
1000ml.

What is the shelf life of the dri-pacs?
9 months.

Do I use cold water?
Use cooled boiled water.

What is MCT content?
60%

Can I use standard Peptamen?
Yes, it is licensed to be used for over 5, however the energy distribution in PJ is specifically designed for children from 1 up to age 10.

What is the fat source?
MCT, Sunflower, Soya oil, Soy Lecithin.

Is soya from a non-GM source?
Yes.

What is the carbohydrate source?
Sucrose, potato starch, corn starch.

What is the whey source?
Cow's milk.

Is it suitable for severe milk allergy?
No.

Is it suitable for strict vegetarians?
No.

Is it suitable for diabetics?
Yes, use under supervision.

Is the product gluten free?

Resource® Optifibre®

I am trying to prescribe Optifibre® and I can’t find it on my computer system/ in MIMS or BNF...
The full name of the product is Resource® Optifibre® – so please search for the product under ‘R’ for Resource rather than ‘O’ for Optifibre.

Is it the same as Resource® Benefiber®?
It is exactly the same as Resource® Benefiber® – only the product name has changed. The nutritional benefits offered by the product are identical. As with Resource Benefiber this new product is available in a 250g tin, but in 16 X10g sachets rather than 16 X 8g sachets.

What is the fibre source in Resource® Optifibre®?
Partially hydrolysed guar gum, which is a soluble fibre.

How do I introduce Resource® Optifibre® into my patient’s diet?
As with all fibre, gradual inclusion of Resource Optifibre into the diet is recommended. Although the amount will depend on your patient’s individual requirements, 3 - 4 scoops of Resource Optifibre® per day are a general guideline for intake. A patient friendly user guide is available. How should Resource® Optifibre® be stored?
Store in cool, dry conditions. Once opened, use within 6 months, replacing the lid of the canister firmly after use. Sachets should be used within 10 days of opening.

Can I use Resource® Optifibre® in children under 5 years?
The product is not ACBS approved for this, but can be used under clinical supervision.

What is the size of a serving?
1 scoop is 5g and 1 sachet is 10g of Resource Optifibre powder

How much fibre is a serving?
4g fibre per 5g scoop

Yes can be heated gently – avoid boiling as this destroys nutrients.

Is Resource Fruit nutritionally complete?
No, like most juice based sip feeds it is nutritionally enriched and is not suitable sole source of nutrition.

Can sip feeds, such as Resource Energy, be used in pregnancy?
Under advice and supervision of a medical trained health care professional, it can be used for nutrition support.

What is the protein base in Resource Fruit?
Resource Fruit is 100% whey based.

What type of fibre is in Resource 2.0 Fibre?
50% fructooligosaccharides (FOS) and 50% galactooligosaccharides (GOS), both soluble fibres.

How should I store Resource products?
Resource supplements can be stored at room temperature in a cool dark place. They are classed as UHT products and therefore do not need to be stored in a fridge. Once opened, Resource should be recapped and stored in a fridge – do not consume the product if left sitting at room temperature for more than 4 hours after opening. Resource range tastes best when chilled.

Can Resource be used in people with Diabetes?
Yes, under medical supervision.

When is the best time to take Resource products?
Best taken between meal times to avoid feeing overfull at the next meal. Sip slowly.

Can I combine Resource in any recipes?

Can Resource sip feeds be used for bolus feeding?
Yes, using a 50ml syringe this can be administered as bolus feeds. Be sure to observe local feeding guidelines for bolus feeding.

Is Resource ThickenUp amylase resistant?
As with all starch based thickeners, amylase can potentially lead to the breakdown of starch. Research has shown saliva and body temperature can cause a bolus to warm and thin and healthcare professionals should consider this effect when prescribing thickeners.

Optifast

Administration and dosage:
For shakes and desserts mix with 200ml cold water. For soups, use 200ml hot water.
4 sachets per day as total dietary replacement. Supplement this programme with 2L of energy-free liquids. A minimum duration of 2 weeks is recommended in order to achieve a significant reduction in liver volume.
